Getting Started

Student Application Use this application for any student who wishes to be a member of a MOUSE Squad. Having them apply creates a sense of responsibility and importance of the tasks they will be involved in.
Student Commitment Contract Agreeing to this contract shows that students are committing to MOUSE Squad and that they understand the expectations and responsibilities of becoming a certified MOUSE Squad technician.
Parent/Guardian Consent Form Use this form to share MOUSE Squad with parents/guardians of a student who is applying to be a member of a MOUSE Squad. Keep everyone involved in MOUSE Squad.
Media Release Form - Adult Forms for an adult or parent/guardian of a student to authorize MOUSE Squad to use photos or videos on our website that include that person.
Media Release Form - Student MOUSE Squad loves to share the great things your squad is doing through a variety of media. If your site does not already have a media release form that allows us to capture MOUSE Squad and promote it, please have your students' parents complete this form.
